DMS Clinical Observation Hours Form

Four hours of observation time is a requirement of the ECTC Sonography Program. The observation must be in an abdominal sonography department. The hours cannot be completed in an OB/GYN setting.

Student: Complete the top section of this form.

Sonographer: Complete the evaluation and signature, and submit the form before the student leaves the facility.
